Stewards of Change Institute (SOCI), working with partners across the US, has developed the first comprehensive conceptual model for surmounting the challenges of obtaining and managing consent to share information across health and human services: Catalyzing Whole-Person Care: Consent-to-Share is the Key – Insights, Next Steps and a Roadmap.The conceptual model is the outgrowth of four years of collaborative work to obtain informed, voluntary consent from people (patients/clients) to share their personal, identifiable, sensitive information across service providers to deliver coordinated, whole-person care.
